The surgery is unable to issue a letter or certificate confirming your vaccination status for overseas travel. Please see Government guidance for further information.

  • Proof of your vaccination status is visible within the NHSapp, which is available for Apple and Android devices.
  • Alternatively, you can call the NHS Covid-19 helpline on 119 and ask for a letter to be posted to you. This must be at least five days after you have completed your course of the vaccine, the letter may to take up to seven days to reach you.

 

New Patient Registration

register_now_blue_signThe doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and must attend a routine medical check-up at the practice.

All new registrations require one form of  photographic identification eg passport or driving licence along with proof of address eg household utility bill and proof of status.

Please bring forms and relevant documentation into the Surgery Monday to Friday 12.00-1.00pm and Monday evening 5.00-6.15pm ONLY.

Medical treatment is available from the date of registration.  Please contact reception for further information.
